Star Awards - Nominate a Star!
We sometimes hear about people or services who go above and beyond in their role within health and social care. We think it’s important to celebrate exceptional service and we would like to pass on your appreciation by awarding a certificate to recognise this.
We will visit the successful staff or organisation to present their certificate, with our thanks for making health and social care a better experience for the people of County Durham.
You can nominate a staff member or service who you think fits the criteria (the criteria document is situated below).

Criteria
Have a look at the criteria before you nominate your star!
1. Your nominated Star must be an employee/volunteer/service or department providing health or social care services in County Durham – this includes charities and not-for-profit organisations but does not include private companies.
2. We cannot accept nominations in a professional capacity from a social care or health service commissioner, employee or provider.
3. The nominated Star must not be related to you.
4. You cannot nominate an employee/service/department more than once for the same event and the event must have taken place in the last six months.
5. We will not accept responsibility for loss or delay or in the incomplete nature of any nominations made using any method, either digitally or printed.
6. Nominees will be notified and awarded a certificate if sufficient feedback has been given.
7. Healthwatch County Durham reserve the right not to honour Star nominations without giving justification or reason; the decision is final, no correspondence will be entered into.
8. Healthwatch County Durham will use all nominated Stars in their promotional activities including printed and digital media; ‘Stars’ may optout of this promotional activity.
9. A Star nomination does not imply that Healthwatch County Durham, in any way endorse a service and this claim must not be made.
10. The Healthwatch logo is trademarked and must not be used without written permission.
11. Healthwatch County Durham reserve the right to amend and/or withdraw the Nominate a Star programme at any time without prior notice.
12. Entrants will be deemed to have accepted these rules and to agree to be bound by them.
13. Healthwatch County Durham reserve the right to add and amend these terms and conditions without prior warning or notice.
