Hospital Discharge Report
In this report we have gathered feedback from patients to find out what people's experiences are of hospital discharge, what plans were in place for them and support received when they leave hospital.
The report covers the responses of people’s experiences of being discharged from hospital. During our research, we visited University Hospital of North Durham and Bishop Auckland Hospital where patients shared their views.
